I am not absolutely sure either, but I think the difference is this.
"Ordinary" polio affects various anterior horns of the spinal
chord, and consequently knocks out the motor neurons that drive
muscles. (there is now some doubt that it is a black and white issue)
Bulbar polio is more to do with the virus affecting areas of the brain -
usually parts of the life support systems in the Brain stem, eg the
Blood pressure control system, temperature control system, the keeping
alert/awake system (the Reticular Formation), heart rates,breathing etc.
It seems to make sense to me that people who got bulbar polio were is
big trouble, and sometimes died. But when they got over this, they
often were not paralysed.
That is all I can think of. Looking forward to hear a more informed
opinion.
