A couple of things I do different:  I have Boot Manager at the beginning 
of the HD (not sure that it really matters) and I have my primary OS/2 
HPFS bootable volume in the first 900 or so MB of the disk.  All disk 
space after that is divvied up between D: and E:.  All volumes are 
logical; none primary.  (I don't remember why I do it that way; somebody 
just suggested it being better and I've never had a problem with it.)

I use the D: and E: volumes for applications and data, respectively. 
There are no other OSes or format types on the drive, just HPFS on C: 
and JFS on D: and E:.

I know this is going to sound strange, but have you tried a different CD 
device that's native SCSI?  I've used the SCSI to IDE adapters in the 
past with no problems, but only on a CDRW that's in an external case and 
connected to a W2K laptop.

Or, how about putting the HDD on one channel and the DVD on the other? 
I've usually done that since the transfer rate of the HDD was so much 
higher than any of the other devices on the chain and as I understood 
it, all devices on the chain would use the lowest common rate.
