I have heard of a few cases over the last thirty years. Like everything
else, it is possible to have factory defects in firearms. The one case
I am most acquainted with was a S&W 39 which was dropped and fired
hitting an officer. None of the ones using a "floating" firing pin
"should" do that but they rarely do. Sometimes it is caused by poor
maintenance, the firing pin becomes dirty/corroded and sticks.
Sometimes the various safeties fail. Rare but it does happen.
