the most common problem is not unloading all forms; if this happens in
the VB IDE then press the pause button and in the immediate window
enter:
for each ff in forms:?ff.name:next

that will list any forms still loaded; add code to unload them on
shutdown.  you can also do a brute-force unload of all forms in your
code with:
dim f as form
for each f in forms
  unload f
next

if that doesn't solve the problem then it could be that you have a
code loop still running or a timer or other control that is still
active.  VB apps end when no forms are loaded and no code is running.

do NOT use the End statement to solve this problem; it will only mask
the real trouble
