I don't remember the exact wording, but there is a hint at the bottom of
the form saying something like "if you answer Yes to any of the
guestions, entry to the US can be denied". So basically it's saying
that don't bother even to read it, just tick all the No's and you're good.

I only saw the English version, btw, and I'm pretty sure many who get it
don't even understand the guestions...
