Ben,

I don't see you list where the following task was performed.  And maybe
you don't need it, but version 4 of PQMagic has this listed as a task when
converting from FAT to HPFS.

11. Copy UHPFS.DLL from the OS/2 Disk 2 to the root of the new HPFS
partition.  (If you have an OS/2 CD-ROM, consult your  IBM documentation
for instructions on creating a diskette from the disk image).

Have you confirmed that the HPFS.IFS is setup in your config.sys?
