You are generous in assuming the Fourth Mis-state has had anything to 
lobotomise since Uncle Walter left. In these troubled times anything they 
MIGHT
have to be lobotomised would probably require the skills not of a neuro-
surgeon, but more likely someone skilled in nanosurgery. A bungstarter, 
well swung and impacting directly twixt and above the eyes, while likely very
satisfying, is too likely to be considered terminal, so we have to keep em 
alive.
