One crack that is around is simply a replacement DUKE.EXE and DUKE.CFG. 
These might be a bit grey in the legal circles...

However, if you can give your customers the following instructions or
organise some one to write a patch program to automate the process that
would be all you need:

Here's how to re-enable adult mode on the Australian version of Duke Nukem 3D,


1. Backup your original copy of DUKE3D.EXE.

COPY DUKE3D.EXE DUKE3D.BAK

REN DUKE3D.EXE DUKE3D.BIN

3. Run the DOS debug program on the
DUKE3D.BIN file

DEBUG DUKE3D.BIN

4 Enter the following debug commands

-E 200 58

-E 210 58

-W

-Q

5. Rename the DUKE3D.BIn file back to DUKE3D.EXE

REN DUKE3D.BIN DUKE3D.EXE

6. Edit the DUKE3D.CFG file

EDIT DUKE3D.CFG

7. Look for the line containing Environment = " " and
change it to Environment = ""

8. Look for the line containing Out = 1 and change it
to Out = 0

9. Save the file.

10. Enjoy the game.
