I have a supply that matches your description.  It is an Ultra 
X-Connect.  It is advertised as 500W.  The cables are all plug-in.  It 
has only one FDD plug.  It is in a heavy, Chrome plated case.

Fry's price was $54.  I only got it because it had a $50 rebate.
