Here's a few more reasons for poor transmission:
Wet cable. Even though modern telephone cable consitsts of plastic insulated
wire, the plastic has microscoptic holes that will allow water to enter and
corrode the copper wire if the sheath is broken.
Wire lead in poor condition. (It's not always 100% cable from the Central
Office out to the customer. There are several types of non sheath wire that
have been used over the years; much still exists. The reliability of the wire
can be of very short life.
