Most likely monitor.

Try bypassing the KVM for a while to see if the problem disappears.
Could be some signal is being bled off by it or an intermediate cable.

Try dropping down to 80 Hz or less, or switching to 1280x960, which is a
natural CRT aspect ratio that 1280x1024 is not. Electronic components
deteriorate with age. Something in the monitor probably has lowered the
max refresh rate tolerance.
