So I've actually notice it happen a few times - but I've only looked into it
deeper this time.

The question is, as you've stated "Why did it happen that time and not every
time"... I guess a first approach would be to keep all packets that come in and
then reply said packets, when we notice it happen again, and see if it happens
a second tiem. Then when it's reproducable, we can debug it.
