I had a couple of IBM PC 330-P75 minitower workstations on a Black Box 
KVM switch and they had no trouble booting without keyboard focus.  This 
particular model of KVM has an option where you can specify an AT style, 
PS/2 style or one other style, as well as a serial mouse, PS/2 mouse and 
a wheel mouse.

I'm quite certain it depends on the KVM itself whether it tells a 
computer that there is a keyboard and mouse there even though they don't 
have the focus of them.  The KVM itself wasn't inexpensive, but then 
again, most everything from Black Box is slightly above market.
