SP-> After I have my door program completed, should I edit the .INC
SP-> file to DECLARE only those subroutines and functions that are
SP-> actually used in the program? And will that result in a smaller
SP-> overall program (.EXE)?
Yes, only include what you have to! :-)
SP-> Also, how would I be able to add to the users remaining time limit,
SP-> such as games like "Black Jack for Time" do?

You will need to write the exitinfo.bbs file (or similarly named door
exit file, which is commonly called the drop file, i.e, door.sys,
dorinfo1.def, chain.txt, etc.) which the BBS will read back in. It's
up to you to know the structures of the BBS door drop file(s) and
create a BBS readable one when your door exits.
All BBS's are different, so you need to consult your BBS docs for
details if necessary.
