Try changing the partition type on ID 02 to 17 or 1b. If it doesn't foul the
Vista loader this may allow os2ldr to treat ID 03 as C: and thus D: would be
where it needs to be on 04.

I wouldn't trust Vista to not corrupt your FAT OS/2 boot partition. It would
be much safer if HPFS.
