It sounds like you may need a template or two ;-)
Set up your document like you want it to be (just the constant elements,
such as the headers, footers, logo's, etc.).
Save it as a template (*.dot).
Next time use File, New to make a document based on a template.

You could use fields for these items, so they will be automatically
updated. Use Insert, Field and select the right type and lay-out of the
field. It might be useful to use the date it was printed in the footer.
To update the filename, remember to save it before printing it!
