I assume at this stage you have run the mbnewusr program? In
/var/log/messages you should see two lines, one for mbuseradd that creates
the unix account, and one for mbpasswd that sets the users password.
At the same time, on mbse's system log and debug logs should be messages
from the mbnewusr program that the account is added or not.
One other thing, if you have differences in the account database between the
system and mbse accounts caused by a crash you need to delete the accounts
by hand to get a clean situation. I fell for that trap too when I was
working on this problem, I had a unix account and no mbse account.
When I was testing I had ask handle off since it's off by default and I
tested it on a brand new FreeBSD and mbse installation. It could be that
there is a problem with that, so can you turn that off to see if it's
working without handles.
If the problem begins when it's turned on again I know better where too
look.
