Russ,
I certainly agree with Richard that you should make no attempt to replace
that tile, but I would also add that you should take some time before you
start grinding again to clean extra carefully the area where the hole is.
You might even want to spray some quick-drying lacquer on the area. This of
course is because the crevices of your epoxy matrix that were exposed when
the tile popped out might have been harboring some larger grit particles
that got stuck there earlier in the grinding process, and you would
definitely NOT want them to make an unannounced return visit now...
