Hammer that point home - THE SINGLE MOST COMMON CAUSE OF LINE FAULTS NOT DIRECTLY ASSOCIATED WITH DAMAGED LINES IS EXCESSIVE LEVEL. Yeah - I yelled. Loud. The optimum point for S/N and error rate both, on an S12 line card is less than 1 db wide. The upside falls off MUCH more steeply than the downside. (It's around -12dbm _at_the_line_card_.) Harmonic distortion and clipping does horror things to eye diagrams - fast.
