I think the real cause is this 
In the first two years a child sets up a pattern of food-in and waste- out.
This pattern is almost impossible to change.
And rightly so - because to change it willy-nilly would do more harm than good.

This pattern is mostly based on breast feeding (food in pattern) and toilet
training (waste out pattern)
