The panel was in place before the accident occured.  When the theatre would
rotate there is no way to block a person from jumping up on the stage and
crossing over to the other side.  Draw a cross and draw two circles around it
(IE like a roll of tape with a cross in the hole).  The "tape" rotates around
the cross.  If you jump up into the "V" that is in between each of the legs 
f
the cross... that is the stage... when the stage is lined up with one of the
theatres there are panels that block access to the other theater on either
side.  But during rotation, those panels do not line up.  So that is when the
accident occured (while the theatre was moving).  She was crushed between the
two panels that form the wall between the stages and the wall between the
theatres.  So the panel was actually the cause of death since she was crushed
between the two panels.  Even after the accident, the "kiddies" that worked 
t
the park continued to do the same thing (hey, I am not stupid like the girl
that was killed... I can get away in time) and so the attraction was closed 
o
prevent any further injuries or deaths by cast members (since they are the
only ones who would be standing at any time during the entire "ride"... but
just like the visitors... they were required to sit in the seats provided for
cast members near the front of the theatre.
There have never been any deaths or major injuries (not counting kids running
and scraping up their knees for example) that occured due to anything except
not following the rules.  When they tell you to "sit down and keep all parts
of your body inside of the ride" there is a reason for it.  Cast members felt
just like a number of people in the public (some still do), that the rules do
not apply to them.  This is why as many "fully trained" cast members have 
een
injured or killed in attractions as members of the general public.  But as we
all know, teenagers live forever (no deaths other than a few heart attacks
have occured in the park to anyone older than 30... most were between 13 & 
19).
