I may not be an expert, but having been adopted from Canada to New York State 
(now I live back in Canada - another LONG story).. I found that if you go to 
Catholic Charities themselves, they are able to give you the proper names and 
addresses to whom you have to write for information.
Should you need me to dig up the name of the case worker I dealt with, I 
ould
be more than please to help in that way and see if there is a way I could 
forward you the forms etc... since I am visiting in New York all the time and 
it may be faster for me to go direct.
