A good read on this is the book abou the Stanford Prison Experiment. It's quite eye opening as to what can happen when required supervision is removed or becomes complacent. In the case of Abu Grahib, the conditions were set from far above in the WH. It was not "a few bad apples". It was a systemic flaw that was designed to produce the attrocities.
