There's a book by the Union of Concerned Scientists on the US nuclear
industry-- fun reading!  One incident: technicians needed to block a pipe
section to do a test.  They blocked it with a basketball, which, being a
little smaller than needed, was wrapped in duct tape.  It didn't hold under
pressure, resulting in a spill.
