Well, even in a public echo I'm ready to take the embarrasment from all
of us :(.  It turns out that all this time I've had my incoming phone
line hooked to the PHONE plug and not the LINE plug.
 
Upon switching everything is working just fine.  And I deserve all the
snickers heading my way!
