I believe he's referring to the 2nd season ep "The Lonely Among Us."
Just before Riker and Worf beam over to the "Yamato," Riker addresses
the Xporter operator, played by Colm Meany, as "Lieutenant." However,
at that time O'Brien hadn't yet been named, he was simply the "extra of
the week," just as he was in EaF as "Con." So wether or not Riker was
addressing "O'Brien" or simply an unnamed Xporter operator played by
Colm Meany is a bit nebulous.

"James T. Kirk" on numerous occasions since, therefore the T is assumed
to be correct and the R was "never really there." Oh well...
