Obamacare was probably part of it, but as you said most of it was
companies cashing in before their patents expired.  Should also note that
some of those companies wait until their patents are about to expire,
then make some insignificant change to the formulation and get a new
patent for essentially the same drug.

In the US you're supposed to have a doctoral level medical degree to
practice medicine.  Yet unlicensed cubicle dwellers make medical
decisions affecting patients every day - by deciding whether or not
(usually not) to pay for some procedure or medicine.

Several years ago I had one of my kidneys biopsied.  The insuror
(Tricare/Medicare) denied payment for every single lab test made on the
