Statistically 16,000 out of 80 Million is less than 1%.  Of that 16,000 I'm
sure many are from elderly people or people who are no longer able due to
mental / comprehension failures to properly follow the directions the doctor
gives them.

I know one neighbor of mine recently perished because she dosed out her meds in
one of those M-T-W-Th-F-Sa-Su boxes and it appeared she overdosed becasue she
took Saturday and Sunday together causing her cardiac meds to stop her heart. 
Granted it was a mistake, her family had little involvement in her life and
when they looked over her meds there were double doses of some meds in every
day.

I blame that, sadly, on lack of supervision as the poor woman was obvious to
anyone who talked to her, to manage her own meds, she could never tell you what
day it was.
