USR use two types of flash-ROM chips, Intel and AMD, and it turns out that
there was a bad batch of AMD devices.

Purely accidental.  Some of the AMD chips had their higher segments
internally flagged as "read-only", so when the larger SDL tried
accessing these segments, the chip shat itself.  RTB replacement is the
only cure here, BTW.  :(
