After further research and testing, it appears the problem is with the DOS 
path translation feature.  If that is disabled (mbsetup 1.4.7 is empty), 
everything works fine.  If you have a DOS drive letter/path listed there, the 
.?lo files are created using the DOS path (with the correct case).  Apparently 
when the DOS drive/path is translated back to unix, it's getting zapped to all 
lower case, causing mbcico to fail to find the file to send.

Since I don't really need the dos path translation feature, I'll just disable 
it until the bug gets squashed.
