Nah the fear starts with an entire city of 10 million people being turned
to rubble in seconds.

The main fear is that the more there are the more likely one will go pop.

I do wonder though why only those with nukes (always obtained under great
secrecy etc) view only themselves as "rational actors".

i.e. take the US & Russian inventory & distribute the excess to
other currently non-nuclear states & possibly the risk might decrease.
