If you were born in Ohio; and born before 1964; you can get your  original
birth certificate.  The board you are sending from is Ohio; so thought maybe
you were born there.  Also you can get non identifying information from the 
State of Ohio as well, if the adoption was final in that State then you can
obtain the  birth certificate from Vital Statistics.
