Re the Simple, I had my friend who's having so much trouble compare your
setup with his.  It's an identical model, and his experience on his
admittedly terrible lines seems to bear out what you say; it needs good
lines.

The only noticeable difference in his setup (apart from Pulse dial) was
that he has S10=14 (the default on most modems), whereas yours is S10=28. 
Is that something you arrived at experimentally, or what?  Does it help?

If I get my new modem soon, we might see how well my DPX596 does at cutting
through the crappy lines from out there ..
