     I think you're right.  His first appearance was in Incredible
Hulk #180-182 (with the bulk of his appearances being in #181)... and
he didn't appear (to my knowledge) again until Giant-Size X-Men #1,
where he was already wearing the more familiar mask.  Wolverine also
fought the grey Hulk during Todd Macfarlane's time as artist on "the
Incredible Hulk".  Both were members of the occasional "New Fantastic
Four" team (Hulk, Spider-man, Ghost Rider & Wolverine). More recently,
the Hulk encountered the X-Men... and Wolverine in a story which saw
the Hulk become one of Apocalypse's horsemen.  They've met as a result
of various Marvel Universe-spanning mini/maxi-series too.
     Wolverine has encountered Captain America on a number of
occasions... whether it be the X-Men Vs. Avengers mini-series, any
mega-crossover mini-series (the "Infinity" group of mini-series,
"Secret Wars", "Contest Of Champions") ...  or the Captain America
Annual #6 (I think) which guest-starred Wolverine.
     The Punisher encountered Captain America somewhere between #200 &
#300 (for some reason, #245 seems to come to mind... but I could be
wrong).  Cap & the Punisher also met in the "Blood & Glory" mini-
series.  Probably a few other places too.
     The Punisher encountered the Hulk just before #400 of the Hulk's
book (as a tie-in to his Mr. Fix-it days)... and later on just after
the Hulk became a "Marvel Edge" book (it tied into the story where the
Hulk lets the Punisher escape...  only to have the Punisher end up
killing Nick Fury).
     Cap encountered Venom in "The Avengers: The Vault" (as it was
originally titled) graphic novel.
     The Hulk encountered Venom in a one-shot produced for a charity.
He and/or Cap may also have encountered Venom as guest-stars in the
various Spider-man titles over the years.
