Basically because the packaging is much simpler when you dont have
to fart around with the quartz window for a real erasable eprom.
The OTP is just the massive volume packaging all the other stuff uses.

Yep, only the quartz window is missing. When the vast bulk of eproms shipped
in say a modem never get returned to be reprogrammed with an eprom change in
the field, it makes a lot of sense to use the much cheaper OTP form instead.

You can also make a case that farting around shipping it back in traditional
eprom format is actually MUCH higher cost, just not quite so visible.
