Capture few diagnostics screens (ATI6I11) and only while online capture ATY11. That should give at least some idea about your line. I'd put my money on your line giving problems but you never know. My only trouble is with some Rockwell based modems, they don't connect at all (either "Unable to retrain" or "MNP incopatibility" error messages) but they connect happily at max speed half a minute later.
