Flow control, almost without a doubt.  What is your init string? 
What setting do you have for flow control in your comm program?  If
your modem is external, it's _possible_ the cable between the modem
and the PC doesn't have a full complement of wires.  RTS/CTS requires
the modem and PC to assert lines, and if the cable doesn't have those
lines, then the RTS/CTS flow control simply won't work.
You must have a wire on pins 4 and 5 on the 25-pin connector and they
connect to pins 7 and 8 on the 9-pin connector, respectively.
