Actually the truth is that you use it because you know he prefers not to and you (and others) do it to bug him. Many people prefer to use their middle names. My doctor uses Michael, although his first name is John; he signs prescriptions 'J. Michael Williamson.' My dad preferred 'Larry' although his first name was also John, he even (finally) got his mother to call him 'Lawrence,' which was his middle name.
