Ok. I have in my hands a EMS frequently asked questions from the Internet 
newsgroup alt.med.ems with a wide description of EMS in USA, Australia and 
Germany.

In Italy the situation is upset. Events are done overall by commercial 
organisation, since they rarely do emergency ambulance service.

In Italy we have following organisations:
- C.R.I. (Croce Rossa Italiana, Italian Red Cross)
- Croce Bianca (White Cross, indipendent from the Red Cross)
- other crosses as Croce Verde, Croce Viola AKA public assistance, 
organisations based upon volunteers, members of Italian Public Assistances 
Society called A.N.P.AS.
- private organisations mainly devoted to events and non-emergency transports 
(hospital to home, for example)
Every region decides how to organize its emergency medical service.
For example here in Lombardia 118 emergency medical number is based on 
volunteer's organisations, without any nurse or medic personnel on ambulances 
(all BLS) except for helicopter and ALS ambulances during the night that 
replace helicopter.
This ALS ambulance here in Milan is composed by:
- driver (volunteer)
- radio operator (volunteer)
- doctor in anesthesiology and reanimatology
- nurse
