In the USA you have EMT-Basic, EMT-D (basic who may defibrillate),
EMT-paramedic, just to mention the first three I have in mind.
By the way I'm BLS and simple trauma management (scoop stretcher, KED,
cervical collar application) trained.

Here in Milan we work for emergency medical service "118" (emergewncy medical
phone number, same as US 911).
EMS at events are just a small part of our work.

Here in Milan we have about 10 EMS volunteer associations, 100-120 volunteers
each, not counting Italian Red Cross. We should give to 118 at least one
ambulance each association, and a driver for medical guard, every evening and
night and on saturdays, sundays and holidays.
For every ambulance we need at least 3 people, 1 for medical guard and 1 as
radio/phone operator who talks with 118 headquarter and ambulances.
I'm usually on duty from 6.30 pm to 8.00 am on saturday night. We have our
duty one day a week.
