I've seen this symptom before.  This comes from something to do with the
Gatesian newline stuff.  The actual ".len" file seems to get an
extra newline indication put in it.  It causes OSLO to pick up an air gap
between all the elements.  OSLO LT then runs out of surfaces with no
indicator that I've seen that it lost half the prescription.
