Wrong...the list you may be reading from might say that, but the data is
not. It's impossible to have identical tags in that database.

It didn't happen because it's impossible to have identical tags in a
database. If they were kept by a serial numbering system, yes, you could
have duplicate tags. As long as they're kept by tag names, it is
impossible to have dup tags.

It is not possible to keep dup tags in a database.

Your only fact is that a dup tag was inserted by hand into your list. It
was not put there by the software that kept that database.
