I have looked through many scopes and looked at hundreds of photos on many
websites.  In nearly every case the focuser/eyepiece is mounted on the side
of the scope.  It seems to me that it would be a little easier on the neck
if it was rotated about 45 deg around towards the top of the tube.  What
are the tradeoffs and compromises of doing this.

I see some scopes were this is adjustable by rotating the whole assembly.
This is always touted as a very desirable feature.  It just seems that is a
variable focuser rotation is a good thing, then a good compromise for a
fixed position would be about 45 deg above horizontal.
